שתי אפשרויות עיקריות עומדות לפנינו:

א. גלולה. ב. התקן תוך-רחמי.

א. הבעיה בגלולה היא שהשימוש בה בגיל המבוגר בעייתי, עקב סיכון יתר למחלות טרומבוטיות [פקקת; אוטם בשריר הלב] עם העליה בגיל, בעיקר בנשים מעשנות או עם גורמי סיכון משמעותיים, כמחלות טרומבוטיות בעבר, היפרליפדמיה [עודף שומנים בדם], יתר לחץ דם ועוד. ישנם רופאים שיתירו במקרים מסויימים נטילת גלולות גם במצבים אלו, אפילו לנשים מעל גיל 40, אך רק עם מעקב רפואי צמוד.

ב. בענין ההתקן התוך-רחמי [=הת"ר] – קיימת בעיקר בעיית הדימומים הבין-ויסתיים אצל חלק מהנשים46 . התכשיר יעיל [פחות מגלולה, אך מגיע קרוב ל99% הצלחה במניעת הריון!] ופותר את הבעיה לטווח של שנים, עד לאחר הבלות47 .

2 . יש לציין שבתשובת הרב זילברשטיין שליט"א מדובר על אשה בת ארבעים ומעלה ; בחלק מהמחקרים דלהלן מגדירים אשה 'מבוגרת' לענין זה כבר מבת שלושים וחמש ומעלה [כאשר העליה בגיל מגדילה את הסיכויים ללקות בסיבוכים וסיכונים אפשריים]. כמו-כן יתכן שיש הבדלים לענין זה בין קבוצות שונות באוכלוסיה. בכל אופן – יש צורך בתשומת לב לעובדה, שלא תמיד יש הסכמה על ההגדרה מאיזה גיל נחשבת האשה 'מבוגרת' לענין הריון ולידה.
