Hello, I am a physician in Toronto and I was asked by a religious patient about using electrical patient controlled analgesia (PCA) machines on shabbos. Would it be preferable for a nurse to press the button to inject the medication or should the patient not have a PCA maching and instead have intermittent pain control – and if so is it preferable to have IV or IM injections.

If the patient has a life threatening disease with pain then: if a non-Jewish nurse is readily available she should be asked to press the button. If a nurse is not readily available and the pain is severe he may press the button using a knuckle.
If the disease is not life threatening he should ask a nurse to press the button. Only if she not available and the pain is severe enough to affect his whole body, then he may press the button using a knuckle.
Regarding the choice of IM or IV injection given by a non-Jewish doctor or nurse, either may be given depending or the medical indication. An IV injection by a Jewish doctor may only be given in a patient with a life threatening disease
