also, with regards to working in a hospital in chutz laaretz where all the patients are non jews, what is the source allowing a jewish doctor who has to work on shabbos in this hospital- something about the way the non jews will treat us, if he doesnt?

2. It is permitted to treat a gentile on Shabbat.

שלום רב. בעקבות שיעור בו למדנו על מציצה בברית מילה, ביקשנו לבצע את המציצה בברית של בננו באמצעות שפורפרת. לצערי, את ההחלטה קיבלנו ללא התיעצות
למדתי בעיון את תשובת הרב הלפרין בענין חזרה מתורנות ע"י נהג גוי בשבת בסוף דבריו מציין הרב בשם הגרש"ז שכל ההיתר הוא בתנאי שבית החולים
