It would be best to ask a goy to tighten the screws. However, if one is not available, thighten the screws with a shinui, for example, using the first two fingers to grasp the screw instead of the thumb and first finger
concerning your follow-up question:
I spoke to a senior orthopedist who told me that the possibility of bleeding when tightening the screws is remote. Therefore we are discussing an action which is not certain and is unwanted ("la psik reisha" and "la nicha leh") and the action therefore only involves a Rabbinic prohibition. Since the patient is in the category of "danger to limb" a Jew may – if a non-Jew is not easily available – tighten the screws in an unusual fashion ("shinui").
